Long-Term Care: Your Hospitals New Priority

From Hospitals and Health Networks comes a story we can only hope plays out in the long run.  With the new penalties on readmissions and the focus the Accountable Care Act has brought to  waste and inefficiency in the healthcare system we need a renewed focus on the long-term needs of the patient.  If someone is discharged today from a hospital or Skilled Nursing Facility is it safe for them to go home with no further treatment?  The continuum of care required by each patient to ensure that recovery and rehabilitation are maximized should be the first priority.  Let’s hope that his becomes a reality in the next few years with the coming Accountable Care Organizations that are beginning to pop up around the country.  Keep your fingers crossed.
